Early results of the functional treatment of patients with intra-articular knee fracture - initial treatment

The basic principle of functional treatment of intra-articular knee fractures is the use of skeletal, ankle, shin in traction as well as fast application of isometric exercises of the thigh muscle in the broken limb. In 2-4 weeks following the fracture, passive exercises of the damaged limb are introduced then active exercises are introduced in weeks 5-6. This helps to set the fragments of the broken bone in place and at the same time enables to achieve a good range of movement in the knee joint. The authors evaluate early results of the functional treatment of intra-articular knee fractures. The study included 4 patients aged 28-69 (x = 57). The condition (after the removal of the traction) was recorded, taking into consideration the strength of the extensor muscles of the knee joint and the thigh muscle, using the Lovett scale as comparison to the strength of the muscles in an unbroken limb, circumference of the shin muscle and the thigh muscle were measured by means of a measure tape in comparison to the healthy limb, range of active movements of the knee joint bend and the degree of knee joint stability. The treatment applied gave positive results. Correct heeling of the bone was observed in female patients. No signs of knee joint instability were reported. The range of bend movement of the knee joint was over 90° and full extension.

Podstawową zasadą leczenia czynnościowego złamań śródstawowych kolana jest stosowanie szkieletowego, nadkostkowego wyciągu goleni i jak najszybsze wdrażanie ćwiczeń izometrycznych mięśnia czworogłowego uda uszkodzonej kończyny. W 2-4 tygodniu od złamania stosuje się ćwiczenia bierne, a w 5-6 tygodniu wprowadza ćwiczenia czynne uszkodzonej kończyny. Takie postępowanie pozwala na poprawne nastawienie odłamów kostnych i umożliwia uzyskanie dobrego zakresu ruchu w stawie kolanowym. Autorzy oceniają wczesne wyniki leczenia metodą czynnościową złamań śródstawowych kolana. Badaniem objęto 4 pacjentki w wieku 28-69 lat (x = 57). Stan po usunięciu wyciągu dokumentowano biorąc pod uwagę: siłę mięśni prostowników stawu kolanowego oraz trójgłowego łydki w skali Lovetta w porównaniu do siły mięśni kończyny nieuszkodzonej, obwody mięśnia trójgłowego łydki oraz czworogłowego uda mierzone przy użyciu taśmy mierniczej w porównaniu do kończyny zdrowej, zakres ruchów czynnych zgięcia i wyprostu stawu kolanowego oraz oceniano stabilność stawu kolanowego. Uzyskano dobre wyniki leczenia. U pacjentek wystąpił prawidłowy zrost kostny, nie odnotowano cech niestabilności stawu kolanowego, osiągnięto zakres ruchu zgięcia w stawie kolanowym powyżej 90° oraz pełny wyprost.
